Centering care in Hispanic Women with GDM

We have really enjoyed working with our Hispanic GDM groups! We just had a reunion last week for a new group.  We had no preterm births or NICU admits and 100% of the women completed their postpartum visit and 2 hour glucose tolerance testing.  We were also able to tie in our population health and patient navigation teams and get all of the women established with a primary care provider to help keep them healthy after pregnancy.
