I work at a: Educational Institution I am interested in Centering because: it is changing the lives of families and fixing a broken healthcare system.
The Essential Elements are the guiding principles that define the Centering model and differentiate Centering from other forms of group care. All Centering practices are expected to adhere to these elements.
ESSENTIAL ELEMENTS OF CENTERING
Health assessment happens in the group space
Patients engage in self-care activities
Each session has a plan, but emphasis may vary
Groups are facilitated to be interactive
There is time for socializing
Groups are conducted in a circle
Group members, including facilitators and support people, are consistent
Group size is optimal for interaction
There is ongoing evaluation

Do health assessments happen in the group space? Is the assessment area is set-up to ensure the feeling of privacy?
Are patients active participants in collecting indicators of their own health? Do patients record their own health information?
Does each session have a plan? Are Centering materials used to plan group sessions?
Do facilitators guide, rather than control, the conversation? Do facilitators refer questions to the group? Do patients voluntarily share information, ideas, personal feelings or experiences? Are group guidelines established? Do facilitators dress casually?
Is there time for socializing?
Are groups conducted in a circle? Are any observers incorporated into circle-up to participate? Is the Centering space private and conducive to group sharing? Is seating around an open space (as appropriate for your patient population)? Are all facilitators, patients, and support people in the circle when formal circle-up of the group begins?
Are group members, including facilitators and support people, consistent? Are facilitators present for the entire session? Are residents, students, and trainees consistent?
Is group size optimal for interaction?
Is there ongoing evaluation of health outcomes? Do facilitators assess the group experience?
