Is your Centering team challenged by staffing difficulties? Does your community agency want to be more involved with an evidence-based model to improve heath outcomes, but not provide clinical services? CHI is addressing these needs through a pilot to include community partners as part of the Centering Team. This model is not unique, as several communities across the country have been successful bringing together community and clinical teams to support group care. CHI is now taking an active role in connecting the dots so that our Centering practices can thrive.If are interested, we would love to tell you more! What: CHI is bringing together Centering practices with community partners who can serve the role of Staff Facilitator or CenteringCoordinator. Why: This pilot will address staffing issues in clinical sites and improve holistic connection with community organizations and the communities served Who: Anyone from current Centering sites, community partners, and/ or potential Centering sites When: Beginning in October of 2022 Where: This pilot is beginning in the great state of New Jersey.
