Paper Spreadsheet/Form to Collect All Data for Online CC

I wanted to share this with everyone. This is a paper spreadsheet that can be used to collect data for a group and then can be entered into CenteringCounts online. My Centering site has used this type of tool since the beginning of CenteringCounts starting with the Excel Beta Version.
As always these are a place to start from and can be modified for your own uses.

You certainly can just put the data directly into CenteringCounts, but again you have to write it down somewhere before you put it in. At least, I do. Or else I am going from the EMR to the online version and back and forth, because my memory is not that good.
Who does it?
With so many groups, this was also created out of necessity to corral all the pieces of data--HIPPA consent forms, photo consent forms, patient evaluations, Facilitator Process forms, schedules, etc. This is what gets turned into the Centering Coordinator after the reunion group.
This folder can be kept in a locked file that is handy for those using it.
I print out there tabs on colored paper and put in a folder with labeled with the due date group

- You usually have to collect the data from an Electronic Medical Record of some kind. In many sites outcome data must be collected from the patients since you have no access to the hospital records. These are forms developed by one of my sites to collect the patient outcome information as well as any info not found in their EMR. - This enables you to collect the data as you go along. You have to write the data down somewhere when you collect from the EMR or the patients, so why not put it all on one piece of paper.
- Certainly, you will start a group online and want to enter the dates of group, the facilitators and can enter any data that you know at the beginning including participants. And can enter any data you collect as you go along, but you usually will still have to write it down somewhere before you enter. Use the paper spreadsheet.
- Then when all the data is collected, one person can quickly enter all of the data into CenteringCounts, especially the outcome data and any other data you haven't already entered.
- The spreadsheet is designed so the data entered is exactly in the order that is in the online version of CenteringCounts.
- You will note that to get all of this info on one page (back and front) or two pages, abbreviations had to be used. These are listed at the bottom with reference numbers.

- It varies from site to site, and what resources you have.
- At my site, we at one time had 24 groups a year and one Centering Coordinator with about 4 hours a week of time. The nursing manager decided the best way to allocate the time was to have all the Staff Facilitators collect the data and then enter it into CenteringCounts. But they were not pulled back to the clinic immediately after the Centering group was done. We had two groups a day--one at 8:30a and one at 11:30 a, then the rest of the day they were responsible for collecting any data, including patient evaluation forms, etc. AT the reunion visit, they were expected to have all the data entered and turn in the administrative folder to the Coordinator. If they didn't, she came looking for them. But they were giving time to do this. We were using the Excel Beta. so I always think it is a good idea to have a "superuser"type so folks have someone to check in with if they get stuck.
- Some sites have the Coordinator collect all the data and enter it all, which I think can be a little overwhelming even if she/he is full-time. Other sites have the Staff Facilitator collect the info and then the Coordinator enters it. However, it works best for your site and staffing.
