Does anyone have an existing list of criteria they use to determine eligibility for Centering Pregnancy (if led by a CNM)? I am using my scope of practice guidelines as a baseline but don't want to reinvent the wheel if others have existing lists. Thanks, Courtney

Hi Courtney, that's a great question. Every practice has it's own inclusion / exclusion criteria depending on its patient population and practice resources. Some practices completely exclude high risk women, others completely exclude low risk women. Some practices hold groups only in Spanish, excluding non-Spanish-speaking women. In our Site Approval process we ask our practices to describe their exclusion criteria, which are most often related to comorbidities or language. When considering practice scale, CHI recommends practices set an enrollment target of 60% of patients that are "eligible" for Centering, but we continue see many practices that exclude half or more of their patients from Centering eligibility. I also think there are political exigencies over patient ownership that play a role here. We need to reframe the conversation. Centering is appropriate for most patients and should be assumed; exclusions, especially for high risk conditions, are generally unnecessary. Instead of excluding patients from Centering, supplement their Centering care with individual visits.

In my experience, many sites start with an exclusion list and as people get comfortable with Centering they negotiate exceptions on a mom by mom basis. Slowly the list of risk exclusions gets shorter. It seems to be evolving and varies by site and by provider type.
